Grane Hospice Care offers a wide range of services to our patients in York. Our staff is highly experienced in providing the best care, both physical and emotional. Our services include, but are not limited to:

• On-Call Hospice Support 24/7: Our team is here for your loved one whenever you need us, day or night, weekend or weekday.
• Pain Management: Our team is well-equipped to administer a variety of medications aimed at relieving pain and discomfort.
• Medication Coordination: It is often challenging to keep medication organized, from dosage to when to take it. Our team helps ensure the proper medication is administered at the right time.
• Symptom Management: If your loved one has been prescribed medication to alleviate the symptoms of their condition, we can help coordinate that as well. We often work with medication to help with anxiety, breathing problems, or chronic nausea.
• Registered Hospice Nurse Visits: You will meet with our nurses to formulate the ideal hospice care plan for your loved one.
• Nurse Aide Visits: Should your loved one need help bathing, feeding, or dressing, Grane Hospice Care provides experienced Certified Nursing Assistants (CNA) to assist them.
• Hospice Team Visits: Our team will provide you with counseling, including an in-depth look at your insurance coverage giving you all the answers you need.
• Volunteer Visits: On an election basis, our volunteers are available to visit with your loved one, run errands, perform small tasks, or just to provide company.
• Bereavement Services: Grane Hospice offers unlimited bereavement services to our patient communities. Bereavement services can begin as soon as services are initiated, or when the caregiver(s), family, and friends of the patient elect services.
