Congestive heart failure is a chronic condition in which the heart does not pump efficiently. Patients with CHF are at risk for an increase in cardiac symptoms and subsequent hospitalizations. Our special cardiac team uses a multifaceted approach to stabilize the patient’s condition, reduce re-hospitalizations, increase independence, and improve overall quality of life.

COPD is a debilitating lung disease that affects more than 11 million people in the United States alone. COPD makes breathing difficult and can cause wheezing, fatigue, respiratory infections, pain, and discomfort. Since COPD is a chronic condition, it requires long-term treatment.
Grane’s COPD health care team works with each patient to develop a step-by-step symptom management plan.
