What is Hospice Care?

What makes someone eligible for hospice care? Hospice Care is an affirmation of life that focuses on comfort, pain-free care when a loved-one is faced with a terminal illness. We concentrate on keeping the patient and family comfortable with a team-oriented approach that includes professional medical, social, emotional and spiritual support during this difficult time. Grane Hospice Care provides comfort for the Body, Mind and Soul through unique, individualized professional care plans that allow you to live comfortably, in your home or nursing community, surrounded by your loved ones.
When is Hospice Care Appropriate?
What makes someone eligible for Hospice Care is when you and your physician decide that palliative/pain-free care and comfort care, is in the best interest of the patient. Grane Hospice Care brings all of the necessary support and comforting care to you and your family, in your home, within a day of the referral to hospice.
Hospice Indicators can include, but are not limited to:
  • Weight Loss
  • Increased assistance with daily activities
  • Increased weakness and fatigue
  • Multiple falls
  • Difficulty Swallowing
  • Shortness of breath or chest pain
  • Continuous oxygen
  • Increase/changes in medication
  • Recurring or multiple infections
  • Frequent phone calls to physician
  • Frequent hospitalizations or ER visits
Who Pays for Hospice?
100% of Grane Hospice Care services are covered through your Medicare or Medicaid hospice benefit. Grane Hospice Care also works with your private insurance company to assure all of your needs are met in the comfort of your home..
How long can you be on hospice care?
A question that is commonly asked is, "How long can you be on Hospice?". Hospice Care is not just for individuals "actively" in the dying process. A person can be on Hospice care with a life expectancy of 6-months to a year. Hospice care makes sure you are as comfortable as possible in your own home or assisted living facility, with the medical and support staff you need in the later stage of your life.
How Do I initiate Grane Hospice Care?
Grane Hospice Care can be initiated by simply calling us. Your community liaison will do the rest. Our experienced Hospice Care team will be able to assist you with all of your questions and concerns. Referrals can be made by families, physicians, social workers, friends, clergy, hospital discharge planners or individuals who need Hospice Care service.
