Routine Hospice Care, Inpatient Respite Care, General Inpatient Care, and Continuous Care
Grane Hospice provides Routine Care, Respite Care, General Inpatient Care, and Continuous Care to meet the needs of our patients and families.
Routine Hospice Care
Routine Hospice Care is utilized most often and focuses on providing physical, spiritual and emotional support to patients and families. Routine Home Care can be provided anywhere that is considered home. This includes your home, assisted living facilities, personal care homes, and nursing facilities. Grane Hospice will provide all mediations, equipment, and supplies relating to the terminal illness or comfort management.
Inpatient Respite Care
Inpatient Respite Care is utilized to provide temporary relief to caregivers allowing them personal time to rest and recuperate. Inpatient Respite Care is provided in a licensed nursing facility for a maximum of 5 consecutive days. Grane Hospice will provide transportation, medications, equipment, supplies needed, and 5 days of room and board payment.
General Inpatient Care
General Inpatient Care is utilized to provide short term crisis management of exacerbated symptoms and/or pain. General Inpatient care is provided in a licensed nursing facility or hospital setting. Patients are re-evaluated every 24 hours to assess continued eligibility and establish length of stay. Grane Hospice will work in collaboration with the inpatient facility to fulfill the medication, equipment, and supply needs. Grane Hospice will reimburse the nursing facility or hospital at a daily contracted rate for level of care and room and board for as long as General Inpatient Care is deemed clinically necessary.
Continuous Care is utilized to provide short term crisis management of exacerbated systems and/or pain. Continuous Care is around the clock care provided mostly by a licensed nurse and can be provided anywhere that is considered home. Grane Hospice will provide all mediations, equipment, and supplies relating to the terminal illness or comfort management. A reassessment for continued eligibility occurs every hour to establish length of time required to manage the crisis.

On Call Hospice Support 24 hours 7 days a week
Pain Management (Treatments to help with pain and discomfort)
Symptom Management (Treatments to aid with anxiety, breathing, nausea and
Medication Coordination (Allows the proper medication to be available
Medical Equipment Coordination (Provides the necessary equipment to provide
the greatest comfort)
Registered Nurse Visits (Scheduled to meet your individualized hospice
Nurse Aide Visits (Scheduled to aid with personal care: bathing, feeding,
dressing and other duties)
Medical Social Services (counseling and emotional support with social and financial issues)
Spiritual Services (assist and support patient and family with spiritual needs and facilitate interaction with community clergy)
Volunteer Visits (Scheduled for companionship, errands and light duties)
Unlimited Bereavement Services (Unlimited bereavement services to all hospice
