Grane Hospice staff are on-call 24 hours a day, 7 days a week. Whether you are in your home, at a skilled nursing facility or assisted living facility, Grane Hospice Care keeps you comfortable and pain-free with a team of experienced and compassionate caregivers. We create an individualized care plan tailored to fit your personal needs. Our hospice care team provides everything from registered hospice nurses and experienced aides to financial and spiritual counseling, and any necessary medical equipment. Your care plan is designed so you can live comfortably, in your own home or nursing community, surrounded by your loved ones. It's comfort for the body, mind and soul.
Hospice Registered Nurse Visits
Our Registered nurse visits are scheduled to accommodate your personalized care plan. Nurses will come to your home to administer professional care to make sure you are comfortable. Your professional nurse care is continual and is scheduled throughout the rotation of care from your nurse aides, volunteers, social worker, and spiritual counselors.
Pain and Symptom Management with Medication
Your pain and symptom management will occur through professional assessments and communication with you and your loved ones. Grane Hospice Care implements a continuous check on your comfort and pain management providing the right medications at the right times.
Grane Hospice Care Services include:
  • On Call Hospice Support 24 hours 7 days a week
  • Pain Management (Treatments to help with pain and discomfort)
  • Symptom Management (Treatments to aid with anxiety, breathing, nausea and other symptoms)
  • Medication Coordination (Allows the proper medication to be available when needed)
  • Medical Equipment Coordination (Provides the necessary equipment to provide the greatest comfort)
  • Registered Nurse Visits (Scheduled to meet your individualized hospice care plan)
  • Nurse Aide Visits (Scheduled to aid with personal care: bathing, feeding, dressing and other duties)
  • Medical Social Services (counseling and emotional support with social and financial issues)
  • Spiritual Services (assist and support patient and family with spiritual needs and facilitate interaction with community clergy)
  • Volunteer Visits (Scheduled for companionship, errands and light duties)
  • Unlimited Bereavement Services (Unlimited bereavement services to all hospice family members)
